Advanced Health Care Planning for Those Involved in the Care of the Seriously Ill

In Hawaii, there are significant opportunities to better honor people’s preferences for care, and we have a unique opportunity to make that happen. We know that the majority of seriously ill Americans, when possible, want their care to occur in their home. But many people choose disease-modifying medications and treatments instead of comfort-directed care, which offers high-quality symptom relief and resolution of spiritual, social, and psychological suffering. Now there’s an option that combines both of these approaches.

Recent data indicates that high-quality supportive care, also known as palliative care, can result in prolonged life and improve or maintain quality of life when combined with disease-modifying treatments. Dr. Volandes’ ACP Decision library of video decision aids supports the process of shared decision-making by helping patients with serious illnesses and their families envision what different treatment options will entail and how they will affect their quality of life.  By actually viewing the treatments on video, they can learn about different treatment options and consider how the treatments align with their goals and values.  Over ninety percent of patients, who watched the videos, recommend the videos to other patients with serious illness.
